Insurance Statement:
While we are not in-network with any insurance providers, we will provide you with all the essential documentation, including test results, billing codes, and receipts, enabling you to pursue reimbursement from your insurance company independently. Payment in full is required at the time of your service.
Late Cancellation and Missed Appointment Policy:
We understand that unforeseen circumstances can sometimes require you to cancel or miss your scheduled appointments. However, to provide the best possible care to all our patients, we have implemented the following policy regarding cancellations and missed appointments:
If you need to cancel your appointment, please notify us at least 24 hours in advance. This allows us the opportunity to offer the slot to another patient in need. If you cancel your appointment less than 24 hours before the scheduled time and we are unable to fill the slot, or if you do not show up for your appointment, we reserve the right to charge you for the full cost of the appointment.
